To get your new DarkSky plate, remember to first go to the DarkSky Utah site by pushing the button below; then fill out and submit the Utah DMV form TC-203. Then come back to this page and fill out the form below. We'll save all of your submission information until we have the 500+ purchase requests that Utah DMV requires. At that point, we'll send you an invoice requesting your credit card information. After payment is received for the initial batch of plates, DMV will start distributing them (including yours!) and we'll start seeing the new plates all over the state.
