Utah DarkSky plate

To get your new DarkSky plate, fill out the information below. We'll save that information until we have the 500+ purchase requests that Utah DMV requires. At that point, we'll send you an invoice requesting your credit card information. After payment is received for the initial batch of plates, DMV will start distributing them (including yours!) and we'll start seeing the new plates all over the state.
